ALL student-athlete medical forms are done online. You may start by going to swol123.net and click on "Join Sportsware". The School ID is SAC DONS (do not put in your school ID number or your webadvisor sign in). Fill out the information requested. We then have to approve your request. Once approved, you will get an email confirmation which will allow you to create your own password. If you do not get a confirmation within 48 hours you should check your spam folders or check in with an athletic trainer for help.
Once you have the email confirmation, you can log in and fill out all the sections and forms required with red asterisks (pink sections in PDF forms). Everyone must fill out the following tabs:
- Under "My Info"
- General
- Address
- Emergency (we need 2 emergency contacts)
- Insurance (upload picture of insurance card as well, this option is at the bottom)
- Medical
- Under "Forms"
- Concussion PDF -saved and submitted
- New Student Packet 2017-18 (Returner packet is not working properly. ALL students should fill out "NEW Student Packet")
- If under 18, they must print out above pdf forms and have parents sign
Please make sure the email you use is imputed correctly. If you do not get a confirmation email within 24hrs, sign up again.
We are able to see who signs up (even if wrong email is provided), when you sign in, when you have failed log in attempts, etc. Yes, you can use your cell phone, but a computer is more reliable.
