- Letter Regarding the Vaccination Checklist and Statement Form -
Dear Child Development Student!
During your program of study, you will be required to spend time in child development centers observing and working with young children. In order for child development centers to comply with the California Department of Social Services, Community Care Licensing Regulations and California law (Senate Bill 792), we must ensure that our students have mandated vaccinations and immunizations that are up-to-date and cleared within a certain amount of time.
All students who perform assigned observations, experiential hours, or laboratory hours in a child development center are required to submit proof of the following records:
- TB Test (considered up-to-date if cleared within 12 months) or Chest X-ray (considered up-to-date if cleared within 12 months)
- Tdap Vaccination (Pertussis) (proof of administered date)
- Influenza Vaccination (Flu) or Waiver (considered up-to-date if cleared within 1 year)
- MMR Immunization (Measles, Mumps, Rubella) (proof of administered date)
- Student ID or Driver's License
Additionally, all students will be required to sign and submit a statement of good health, stating that you can physically, mentally, and occupationally perform the duties necessary during your work in the centers. Some examples of these duties would include helping a fallen child get off of the ground, carrying an infant, leading a small group of children in circle time, standing and taking notes during an observation, interviewing parents, and designing and creating enrichments for a classroom. The California Department of Social Services, Community Care Licensing requires this statement to ensure the safety of both the children and you as a volunteer student in the classroom.
Attached, you will find the Vaccination Checklist and Statement Form. Please complete this document and attach copies of each of the required records. You will need to submit this documentation no later than the 3rd course meeting (typically 3rd week of class). Without this documentation, you will be dropped from the course. In addition, without this documentation, you will not be allowed in the center.
