Copies of the following documents MUST accompany your completed application
- DD-214 Separation Report
- 1040 Income Tax Form or other income verification (i.e. public assistance or complete attached Income Declaration - see application)
- Transcripts - High School or most current college transcripts (if available)
- Current Photo ID (i.e. driver license, service ID card)
